Toni, I think that you have no real choice except vet care.

If the thinness is that his ribs are showing (esp. if he is
wider around the abdomen -- hence pear shaped),
combined with the scruffiness and paleness I think
that you have to consider that he might possibly have
anemia secondary to adrenal disease. If so, that is a
serious problem. You can read about it in the Archives
by searching under terms like

Your description of his breathing problems also worry
me and would lead first to a chest x-ray if any of our
crew sounded like that, and then likely to further tests.

I am sorry to have to tell you when you are maxed out
that this description sounds like there is no choice but
vet care, but honestly there simply is no responsible
replacement for qualified veterinary care. That is just
reality.
