Subject: [ferrethealth] Franklin still ill- when to consider exploratory? And other suggestions wanted!

Franklin is still not doing well. He is very skinny still. He feels
frail and limp when I pick him up and it really breaks my heart every
day to see him when he used to be soooo frisky and playful and just
generally a very bad boy (getting in to everything and anything) just
last summer. This has been progressively getting worse for over 6
months now.

He also goes up to food bowl and looks in and then seems to decide not
to eat. He will eat duck soup I made but is one of the first to
finish. He seems dehydrated to me when I pinch his neck but Dr. X
said to check his gums also and they are fine.

He was doing really well last week, he was even dooking and dancing a
lot. This week he is back to his tiredness. I have no idea what the
difference was.

He is on:
Biaxin
Epogen injections 1x/2days
Prednisolone
Pepcid A/C .5ml / 2xday

His aspirations came back:

Spleen: Enlarged spleen. Unremarkable; lymphoid hyperplasia; mild
extramedullary hematopolesis, possible splendid congestion.
Reactive hyperplasis is a nonspecific response to chronic antigenic
stimulation. extramedullary hematopolesis is a common finding in the
spleen. There is no evidence of inflammation and lympho- or
myeloproliferative disease in this smear.

Liver: Cannot verify source of aspirate due to absense of hepatocytes.
Consider reaspiration.

Ultrasound results: Hyperechoic enlarged spleen. Coarse hyperechoic
liver. Periportal lympadenopathy. Mildly reactive mesenteric nodes.
Abnormally shaped right kidney. Adrenal gland hypertrophy (which is
odd cause he had adrenal surgery 1 year ago).

Urine culture: negative

Blood:
Hemoglobin (Low) 11.1
Hematocrit (Loe) 29.7
RBC (low) 5.28
MCHC (high) 37.4
Platelet count (low) 85
Cjloride (high) 116
Globulin (low) 2.6

Also he was on isoflurane at time of blood draw so the vet seems to
think this may account for anemia?

ADV Saliva test- negative

Dr. X is going to recheck his blood in 3 weeks and I may at that
time consider exploratory surgery...I don't want him to get worse
still and not know what is wrong. I'd rather go in sooner while he has
a good chance of recovering from surgery. I know in my absolute heart
something is wrong, but I just don't know what it is or how serious.
