The digestive process of a ferret is about 3-4 hours from start to finish. If River has earlier stage insulinoma, then it seems possible that his activity level is good most of the time, when he has food in his system to bring up the BG level, but at the end of that 3-4 hour period, when the food has been metabolized, then the BG levels could sink quite a bit again. Insulinoma is a progressive disease, so in the earlier stages activity levels might not be noticeably different, but if you do not treat it then it will continue to get worse. The BG test is the best way to diagnose and monitor its progress, and readings of 58 and 41 seem like a pretty sure sign that he does have insulinoma. Unless your vet has another idea as to what is causing the low BG levels, River should probably be on Prednisolone at this point.
