Okay, this is wild guess and nothing more than that, but is it possible that the insulinoma
cells which were vulnerable to that particular chemo approach already died off, but that not
all of the insulinoma cells were vulnerable, so that what was left is now taking off?

I vaguely recall reading a press release recently on one type of malignancy(but can't recall
which one) which was about genetic variability among tumor cells, and if my memory serves
(and don't count in that during tree pollen season) there was a decent bit of variability, and
the authors conjectured that the same conditions which permit the development of some
malignancies might also permit an increased rate of mutation in those cells. Anyway, if that
recollection is right then it makes the "resistant cells remaining" postulation more likely.
