You say he has had two surgeries to remove the tumor, but has he also had a partial pancreatectomy? According to Dr. Bruce Williams, it is much more effective to remove part of the pancreas, not just visible tumors, when doing insulinoma surgery:

Two drops of Proglycem is also probably not enough, especially given his weight. Check that dosage with your vet.

I have a 6 year old male who has had insulinoma for 4 years, he had 2
surgeries to remove the tumor, but it keeps growing back. Its been 3
years since his 2nd one. He has been on pred for about 3 yrs but I dont
think its doing much good so I started giving him about 2 drops of the
proglycem, I dont know if it does any good either. The problem now is
that he has gained so much weight 6.5 lbs and he doesnt have much use
of his back legs especially when he goes potty he just kind of does the
splits and goes down. Is this because of his weight or something elts ?
He has never had any kind of seizure, no drooling. He has staired off
into space some. He sleeps a lot and loves to eat and get a bath. He
drags himself down the stairs just so he can fall asleep near me while
I am working. What can I expect or do for him. I know he is probobly on
his last year with me.
