Subject: [ferrethealth] Bear (mitral valve meds, Pimobendan vs digoxin, kidneys)

Hi, all.
You might remember my Bear from about 18 months ago when he had major
kidney damage occur after pancreatectomy-induced high blood sugar.
His kidneys actually recovered fairly well; his BUN and creatine have
consistently been in the "higher end" of normal for the last year. (I
can't remember the exact numbers off hand, but that's not the point
of this email anyway.)

Recently, we discovered Bear has some critical heart issues. (See
below.) The veterinary cardiologist we visited last week suggested
the protocol below, but did not mention Pimobendan. (I've called back
and left a message asking about it.) If any other medical
professionals (including smart laypeople!) here would like to look at
his protocol and diganosis below and make alternate suggestions,
please feel free! I'm also wondering about the Enalapril, since I
saw it mentioned in the archives as being "less kidney friendly" than
other drugs.

Bear is on day 5 today of the protocol below, so he's just started
his .25 dose of enalapril.

Bear is approximately 7 1/2 years old, but could be older. (He was a
rescue through Pam Sessoms and TRIFL, so we aren't sure how old he
was when he was found 7 years ago.)

"Diagnosis and Recommendations:
Severe mitral valve regurgitation with secondary significnat left
heart enlargement
Mild tricuspid, aortic and pulmonic valve regurgitation
Pleural effusion
Congestive heart failure
Historic renal disease
Recommendations:
continue the lasix (10 mg/ml) at .1 ml twice a day (can increase to
3x a day)
Start Lanoxin (digoxin) (.05 mg/ml) at .1 ml once a day
Start Enalapril (reformulated to 1 mg/ml) at .1 ml once a day for 3
days: .25 once a day for three days. Then .5 ml once a day.

Recheck in 2 weeks for post-dosing digoxin level and chm profile.
Recheck echo in 4 mos."
