It sounds like this could have been a seizure caused by the insulinoma. Insulinoma can progress at different rates, and in it's late stages the progression can be fast. IF it was a seizure then the excessive insulin in the blood ran out of glucose to attack, and so it went after the brain, and may have caused some brain damage. If it were me, I'd have his blood glucose levels monitored fairly closely, and consider increasing his levels of meds, especially the diazoxide, pending your vet's analysis.
