With our Nanook, the FIRST sign of adrenal was a total blockage. They operated immediately, but since both glands looked normal they let them be. They pulled him through the blockage and we started Lupron and propecia. About 6 months later he had another surgery where they removed one gland, which was very obvously 'bad'. This was done by an expert ferret vet, who said the remaining adrenal was fine. After that surgery nanook was peeing like a champ... About 3 weeks later he started again with difficulties urinating. About 2 months later we took in for another surgery, and the remaining adrenal was definitely bad, and removed... I guess this is a long way of saying that yes, blockage can be the first, and only sign of adrenal, and you can't always tell the inside of a gland by it's cover... We're currently nursing our old man, somewhere between 7-10 years old, who had adrenal surgery on Monday because he was blocking..
