Some possibilities to consider or eliminate:

Blockage is possible.

So is HYPERtrophic cardiomyopathy
(which typically needs ultrasound to be found).

A/V Node block is also sometimes missed.

Some lymphoma cases hide themselves. When
we had one the only thing they could think of that
they didn't check over that year was the marrow.

ADV or toxoplasmosis which has gone to the brain
can be missed at times.

Yes, depression also is possible, so is infection, and
I am sure there are a number more possible things.
