Do you always provide food when you give the Pred?
That is important to protect the stomach.

There are at least two strong possibilities here. The
first is that the insulinoma itself is worsening. Nausea
is common with insulinoma whenever is worsens.

So, one of the first things to do is to have the vet check
the blood glucose again because the meds may need to be
stepped up, timing altered, etc. Are the meds at least
twice a day (or three times with even spacing). Is the
Pred Prednisolone rather than Prednisone. Prednisolone
skips a liver processing step so if the liver is at all
compromised it will be more effective, sometimes
greatly so.

Forget the Pepto.

Talk to the vet about adding Carafate. The Carafate MUST
not be given at the same time as the other meds or food, so
give it maybe a half hour before the others. Ask for the
pediatric liquid preparation of it that all pharmacies seem to
have as a prescription item.
