Couple things come to mind. Unless this is a cystocentesis I
wouldn't always hold a lot of faith in it. Esp when the sample has no
blood. Also if urine is not spun immediately and has been sitting
any amount of time you might get Calcium Oxalate crystals as long as
you aren't looking at a lot of them. The combination with Struvite
also makes me wonder. I would recheck do a cystocentesis UA and
culture. I would also ask the vet to spin some and look at it before
sending out to see if they see any crystals. This might help clarify.

If he isn't responding to antibiotics maybe there isn't anything to
really respond to and you are looking in the wrong place.

I haven't followed all of the notes. Holistically you could try
cranberry juice for a bit. Maybe 1/2 teaspoon once or twice a day.
It is a natural antiseptic for the bladder.

I don't know if this helps or makes matters worse but hope it just
gives you some options.

The newer diets that Sukie mentions Royal Canin SO is designed for
both types of crystals in an effort to make it all easier. The
Potassium citrate hasn't been found to work really well and animals
that have these stones removed. often have them recur.
