Sometimes it is hard to tell which is the problem since they can strain
for either or both. A swollen prostate can affect stool elimination
for example.
If the stool looks perfectly normal when he manages to go, I would
suspect that the problem may be urine (i.e., bladder and/or prostate)
related. There are a number of things that could cause straining such
as adrenal disease (prostate enlargement), bladder infection, uroliths
(stones), etc.
Karen is correct in that sometimes they do manage to pass urine and
then suddenly block.
If he is indeed 'constipated' I would worry about an obstruction.
Any other symptoms i.e., coughing/gagging, vomiting, lethargy, loss of
appetite, abnormal looking or scanty stool, etc.?
An assessment by a vet would be in order.
