I'd start by treating the adrenal disease. Adrenal disease affects muscle tone so it may be that he can't support himself anymore while going and that's why he messes himself up. IMHO the licking part is just him trying to clean himself up, because ferrets are actually very clean and just don't like having urine or feces on them. From your post, he doesn't sound like a ferret in pain, just an older ferret.
Is it possible for you to get a second opinion? Adrenal can also be uncomfortable if not properly controled. This doesn't mean the ferret is in agonizing pain, but it does affect their quality of life.
