Have you had his heart evaluated? The VD looks like it is quite large and it seems a bit rounded on the lateral. An Echo on this might be a better step if not already done since heart can cause weakness and tiring easy thus making it harder to do stairs. Just a thought. It is hard to see from this photo I can't get in close enough to look even further at the spine. Good Luck hope you find the answers you need.
