Hi there, I read your post, I didn't see that an ultrasound was done.
Having the same situation at my house before, it turned out to be stones, (some of which can be as small and sand like, another time crystals and a third time a blood clot.

Frrets had been on the same routine as you outlined (lupron, melatonin implants, propecia, Baytril for the possibility of infection and to keep the temp down etc..)
The one with the blood clot was resolved with an agressive expression of the bladder (by the vet) and had a good outcome.

The other two were having infrequent stones (or a sand trail)
travel up the urethra and cutting off the ability to pass the urine.
When the cathereter was successfully put in the stones were pushed back down into the bladder or enough that urine could get passed. Eventually the outcome is not good. Lupron and propecia will help to keep the prostate out of the way, but it doesn't do anything to deter or reduce the stones.

I am hoping that your outcome is brighter, sounds like you are exhausting every possibilty.

The outcome largely depends on the TYPE of stone with
the approaches for some types exactly the opposite of
the approaches for some other types. Urine pH measurement
is important and if the uroliths can be analyzed by a lab
which specializes in uroliths then so much the better.]
