One possibility is nerve damage but I suspect
a more mechanical cause.

I think that your description of the anus was
actually a bad rectal prolapse and that there
may be damage to the anal sphincter. In such
a case the vet may want to first try what is
called a "purse string" which is basically a
few stitches that stay there for a while, but
if there is more damage from a prolapse or
if there is an anal fissure (tear) then the vet
may need to do more to hopefully return
control.
