There are actually two anal scent glands, one on each side of the rectum. These glands can become impacted/infected, in which case you will see swelling and an increase in expression. Cancer of these glands is also possible (it is rare but does occur, I've had one with cancer of the anal glands myself), and the same symptoms will be seen.

In either case, a vet visit is warranted.
