I contacted the people my vet had been in contact with at Peptech in Australia and received a reply today:
...The veterinarians in the US can contact us directly, so you may give them my email address and Katie Yeates' email address (whom I have copied in on this email). As far as we know there is no longer any specific paperwork that is required to import Suprelorin implants in to the US, as long as they are small amounts of the product sent directly to a registered veterinarian, intended for direct usage with specific patients.
