Your Vet should be able to determine this through a skin biopsy-
more than one.
I would suggest though that to get too the bottom of his problem that any meds you are trying on him should be discontinued for at least a week with Drs approval.
Pred can seem to help, but pred alone would not be a standard protocol; usually in combination with an immuno supressor.
