My vet also required the TN Panel before he would treat Pop, even though it was basically obvious from his signs (rapidly advancing hair loss and blocking prostate) that he had raging adrenal disease when he came to us after being abandoned. I had no choice but to do it his way or to find another vet. Since he already had the Deslorelin implants that he had ordered for another of our ferrets, I dished out the extra money (more than your $130) and when the TN Panel came back and confirmed raging adrenal disease, Pop got his Deslorelin implant. Some vets will not waver from protocol in diagnosing before treating, and perhaps that's a good thing in some cases, though maybe not in others when time may be of the essence. If you value your vet, then pay the $130 to get the test and then go from there, since your Cocoa does not seem to be an emergency situation. It should only take a week or so.
