Thanks to all who have replied thus far. I have to decide soon if Sabrina should get insulinoma surgery, or if I should continue to treat with prednisolone. Her daily dosage isn't too high yet (a little over 2.0 mg, in total, divided by 3 doses daily), but the disease is progressing and so is her need for higher doses.

The main question has not been whether or not she has insulinoma, which she clearly does, but rather, whether her very high insulin level (2,755) might indicate that we are dealing with an aggressive carcinoma rather than just 'normal' islet tumor insulinoma. In other words, is there a correlation between extremely high insulin levels in the blood and the TYPE of pancreatic tumor(s) that we are dealing with? This may help with my decision whether or not to go ahead with surgery.
