Subject: [ferrethealth] insulinoma surgery for seven year old Hilbert on Wednesday

His meds simply need increasing way too often. The addition of
diazoxide helped, possibly more so because he personally tends to eat
only 2 to 4 times in any day and it evened his blood glucose out more,
but his insulinoma just is not very responsive. We've been in this
situation before. Sometimes lymphoma or carcinoma located in the
pancreas can do that and then surgery really isn't an option. Usually
then the CBC, or Chem Panel, or both blood tests will be substantially
off. His levels are really very good. He also had his heart image
fine. So, it may be simply that a lot of the pancreas is involved or
Hilbert may have a very large insulinoma. For either one surgery can
help, so that is what will be happening. Meanwhile, his Prednisolone
doses (given three times a day) have been increased, and we are
feeding him many small meals.

I've been asked how Mornie who has a downright huge right adrenal
tumor with rapid onset, probably carcinoma, is doing. The size and
position made surgery not really a choice. She tends to have trouble
with surgeries, so something as massive as an operation that would
require ligation of the Vena Cava just is not in the cards for her.
Instead we opted for a deslorelin implant and she is just thriving on
it. She's still very bald but her body shape improved a bit and her
activity level is very high. BTW, she has also developed a sebaceous
epithelioma on her face between her ear and eye but it does not seem
to bother her much. Morn is in her 6th year.

Both individuals have neural crest genetic variant markings, and both
are also prone to cystine uroliths which have been able to be
controlled with a diet that includes no more than 35% protein.
