I also use oral melatonin as a 'preventative' and have found, unfortunately, that it really doesn't do much. If you insist on not treating with a stronger medicine, then I would suggest that you at least get a melatonin implant. They are inexpensive and last 4 months and work far better than oral melatonin. By the way, if he is having an enlarged prostate, which can be very dangerous, then even the melatonin implant may not be enough at this stage of his adrenal disease. For what it's worth, I've had the best results using the Deslorelin implant, but I'd really suggest that you at least give him a Lupron Depot shot.
