Hey Maureen
Its time for 2 things. One time to recheck the concentration of the pred you are using, so we can see
how much pred Rufus is actually getting. The second thing is to check his glucose level. If it is still low
then it is time to increase his pred dose even more.
Hope that helps,
Jerry Murray, DVM

Tressie,
I guess it wasn't clear in my post but I understand the problem with
reaking the pills compromising the effectiveness of the medication. This
s why I was seeking alternatives for administration. Sorry if this was
onfusing in my original post but thank you for taking the time to respond.

Lucie

Hi Stacey,
It does sound like megaesophagus.
If you haven't already seen the posts by Dr. Heller, you can find them:
http://pets.groups.yahoo.com/group/ferrethealth/message/9622
http://pets.groups.yahoo.com/group/ferrethealth/message/9656
If your vet does the barium swallow Dr. Warren recommends it will at least
onfirm whether megaesophagus is what you are dealing with.
Unfortunately, if that is what it is, it's a horrible condition and the little
ne I was caring for had to be euthanized when the condition progressively
orsened. We did a lot of what you have already done and finally did the barium
wallow, which confirmed our worst suspicions.
Tressie
[If it is MegaE then there are ways to
educe the effects of the problem for
while:
http://www.miamiferret.org/megaesophagus.htm
How well they work will vary among
ndividuals.

My apologies to Dr. Heller, the second reference should have cited Dr. Heller
OT Dr. Warner.

Thanks to everyone who listened & has worried about Rufus with me. We are not
oing to do the combo shot I mentioned before as pretty much everyone (vet &
egular fert owner) seems to think the only outcome would be death & it might be
orrible. So I'll pass on that.
n the advice of some vets on the fhl, I upped his pred just a hair from 0.1 ml
x/day to 0.15 2x/day two days ago - our regular vet was ok with this as well.
This morning, he exited the cage under his own power for the first time in 2 or
weeks!!! Woohoo!!
We figured out a way to get him to eat more; it's nothing complicated, we just
ave to stick a pile of food in front of his face. I'm wondering if all the
eizing has caused neurological problems & he simply forgets where he is? Of
ourse, he is an old guy & blind. I'm not sure I'd know where anything was to be
ound if I was him either. :) He's also finding his way to the litter part of
he cage much better now (we had to cover the whole lower floor of the cage with
uppy pads & put the litter box on top of them). He still doesn't make it into
he box but at least gets near it & there's not been any poop on his body
nywhere either. And that's good because he really is not comfortable with
aths.
nyway, there you have it: an update! And it's great news!!
Maureen
[It sounds like your vet might be worried
bout Prednisolone side effects. Unlike
any other species ferrets are NOT usually
rone to such problems. An exception is
risk of ulcers, but poorly controlled
nsulinoma can also be a cause of that, and
here are things that can be done to prevent
hat problem such as always feeding when
iving Prednisolone. See past posts from
erret expert vets such as:

ttp://ferrethealth.org/archive/YG1140
ttp://ferrethealth.org/archive/YG4287

Many people here are not "regular ferret
wners". There are ones here with decades
f experience, ones who organize ferret
onferences, ones who have helped with
eterinary research papers, ones who have
ared for hundreds, etc. but there are also
ewbies to ferret healthcare so the knowledge
ase varies a lot among the member
on-vets.

I was very glad to read that Rufus is doing better. I would have the
et check his blood glucose and if it is still significantly below
ormal then I'd ask about increasing the pred dosage a bit farther.
ou may still see more improvement.
At age nine I wouldn't worry about consequences of long term pred use.
I'd just worry about keeping Rufus as happy and as comfy as possible
or as long as he lives.
All the best,
aity and the terrific trio

Good news that Rufus is improved, however, my understanding is that the range
or prednisolone in an insulinomic ferret is 0.5mg/kg - 2.0mg/kg every 12 hours.
nd if you are giving him a concentration of 5mg/5ml that is still an awfully
ow dose he's getting.
I believe one of the vets who posted in response to you recommended increasing
he dose to 1mg/kg.
Formula is: (Weight in Kg) multiplied by the dose (mg/kg) divided by the
rednisolone concentration (mg/ml) which would equal the volume of mL to give.

t(kg) x 1mg/kg
______________ = (X)mL every 12 hours
1mg/ml

ou will need to know his exact weight. For example if he weighs 1kg and you
re using 5mg/5ml concentration this would work out as 1.0ml every 12 hours as
er the above forumla. At minimum with the low end range being 0.5mg/kg he
hould be getting 0.5ml every 12 hours at a concentration of 5mg/5ml.
Hope that's clear.
