Sorry to hear your little one is so sick.
Pred would not make the lymph nodes swell more, it is an anti-inflammatory and is used as supportive therapy for lymphoma.
It does sound like what you are dealing with is an aggressive cancer - probably lymphoma concomitant with an infection of some sort. Needle aspirates can easily miss cancerous cells.

Is he on pain medication? If not, should be to at least make him more comfortable while you decide what to do next.
I think if you want to know definitely what you are dealing with, you will need to do a biopsy on either the growth on his foot or an entire lymph node.
