My vet recently did this for us, I am not party to what forms he needed but Peptech talked him through it and he said it was no big deal, not a lot of paperwork to fill out.
He said there were several emails back and forth regarding setting up an account and we had our implants in less than 2 weeks with no problem.
The cost for us for 6 implants and shipping (shipping was $80.) was $308. That will vary slightly depending upon the current exchange rate of course.
