Antibiotics don't work unless taken as prescribed! And even worse you are breeding bacteria that becomes more and more resistant to antibiotics if you mess with the prescribed doses.
Antibiotics are not designed to be used sparingly. Give as directed or you may be creating more of a problem than the one you are treating for.
Very few medications have actual 'sugar' in them, its likely flavoured with an artifical sweetner. Check with a pharmacist and ask your vet whether it in fact has sugar in it.
And even if it does, unless he's insulinomic, it is not likely to harm him for the short time you are giving it. The benefits of the antibiotic far outweigh the miniscule risks of developing insulinoma.
