Subject: [ferrethealth] Bad reactions to Iso and Sevoflurane: ideas?

Hi everyone,

Little Bad is a 4 year old late-alter girl with insulinoma. When she
was spayed at about 18 months of age, she nearly died on the table
under Isoflurane when she quit breathing, turned red, and began to
have heart problems. She has recently developed apparent insulinoma
and my vet tried to do surgery last week, this time using Sevoflurane
since it should be safer. The tech didn't even get her belly fully
shaved before she quit breathing, turned red, and started having heart
problems (we aborted the surgery attempt). In both cases (the spay
and the insulinoma surgery attempt), her heart responded to
epinepherine, and she was also given dexamethasone and hetastarch to
combat the reaction. She was premedicated with atropine and
hydromorphone.

Immediately after last week's failed surgery attempt, she had a heart
echo, which showed a structurally normal heart. She had a full
CBC/chemistry panel before the surgery, and everything was fine except
for the BG. Her insulin level, though, is off the chart (>1440
pmol/L).

I also have Little Bad's father, Ogre. He had the same reaction when
under a fairly light dose of Isoflurane for a radiograph in 2008. He
was not premedicated with atropine or hydromorphone, so assuming the
reactions are the same, these related ferrets seem to be reacting to
the Iso/Sevo.

Does anyone have any ideas about, or know of any good contacts for how
we might be able to get Little Bad safely under general anesthesia to
accomplish this insulinoma surgery? I know I can maintain her on meds
and skip surgery entirely, but she's young and seemingly otherwise
healthy. In my experience, surgery is much more effective.

My vet is excellent and very experienced. Most vets these days seem
to deal strictly with Iso/Sevo for general anesthesia. I'd appreciate
any thoughts about alternatives suitable for a full-on abdominal
surgery.
