He may have developed an infection on top of everything else this poor little guy has been through. And if he's not eating or drinking I suspect he may also be dehydrated. If you gently twist his scruff does it snap back or remain tented or slowly goes back? If it doesn't snap back he is likely dehydrated, which will add to his discomfort. Ask your vet how to show you to do sub-q hydration at home if you haven't learned yet.

He should be seen by the vet again to determine what is going on. Infections can flare up fairly quickly.
