According to Chen (2008) "the recommended dosing for diazoxide starts at 5 to 10 mg/kg by mouth every 12 hours, and is gradually
increased to a maximum of 30 mg/kg every 12 hours if lower doses do
not control signs adequately. Adverse side effects typically include anorexia and vomiting. Additionally, this medication should be used cautiously in patients that have renal disease or congestive heart failure because it can cause sodium and fluid retention (p. 114)."
Generally the dose of pred is decreased when diazoxide is added. Your vet has already advised you that he is receiving the max dose of pred.
The hind end weakness is more likely due to the disease and not the result of the pred itself. If he's walking better since starting the diazoxide then it is helping to better medically manage the insulinoma than the pred alone.
