Yes.

The ONLY way a swelling in that location could be an anal sac would be if the situation were very severe with a rather internal drainage that had formed a fistula and therefore was depositing downward through the fistula. That is a very rare possibility

The vet will check for several things. With that location the bladder will be among the things checked.

To me, a large apocrine cyst looks most likely from the photos, though maybe an apocrine tumor, but it is not possible to diagnose from photos and a vet appointment promptly is needed.

An abscess/boil can also look like this but if so it is huge and sepsis would be a risk so the vet would have to tackle it.

What adrenal disease treatments are being used? There does not seem to be decent control and people here can advise on types, combination, and timing for treatments.

If the adrenal disease is not well controlled an if this is high enough that the nipples still come down that far then a mammary swelling secondary to the adrenal disease is possible, though it looks more apocrine in the photo to me, and swollen breasts with adrenal disease typically happen with multiple ones and usually are smaller and differently shaped without the dark area.

Hopefully, by now this ferret has had veterinary treatment, is doing fine, and Kim will update.
