To add to what others have already posted, I have had similar experiences, 3 cases come to mind, where all the tests came back with nothing obvious. However, adding pain medication did seem to make a difference. Still it was hard to get a definitive diagnosis. Unfortunately, in these cases, in my experience, we didn't find out what was wrong until a necropsy was done.

And in the 3 cases that came to mind, they were entirely different conditions. One had a ruptured spleen that had actually separated completely into two distinct pieces at some point, but wasn't evident in the x-rays. Another had pancreatitis. And the third had adrenal carcinoma.

A surgical exploratory may be the only definitive diagnostic procedure, although if its in the brain or spine, even that may not find it. Still an ultrasound is much less invasive and could nail it.
