It is unfortunate that the vet did not do a necropsy, which would have confirmed the cause of death. Not knowing would no doubt be contributing greatly to your pain of losing him.
It certainly could have been some sort of vascular accident, e.g., thrown clot, embolism, stroke since these types of things can come on quite suddenly without much warning.
It is unlikely that his condition was related to the adrenal disease.
I've been there and know the anguish, confusion and guilt that comes with feeling that somehow I missed something. But sometimes there are no symptoms to alert us that something is terribly wrong.
