I would definitely consider an ultrasound to go along with the bloodwork in trying to diagnose this problem. It can detect enlarged organs, tumors, and even stones.

One specific question: What as the bilirubin level? A high level here could suggest something else.

You might also try feeding the soup a little more frequently, if at all possible. And possibly adding milk thistle for liver support. But check with the vet first on that. And certainly he could use a probiotic mixed into the soup at least once a day, given that he’s on antibiotics, which will kill off his ‘good’ bacteria, along with the bad. That could be a direct cause of his bad stools, and poor digestion would also contribute to his weight loss, which would contribute to his lethargy...
