Its not likely a placebo effect, you are correct in that. Without actually testing his blood glucose before and after the treatment one can't say for certain whether his blood sugar is stable. Insulinoma is a fluctuating disease and this may be a high before a crash.
Please have his blood glucose checked regularly if he stays on this treatment to monitor how he is actually doing. Your vet should not have an issue with that.
