We use the Pedia-Pred strength, 6.75/5ml, though we're now using generic (alcohol / sugar free) predmisolone.  The highest dose we'll use is 1ml bid or .7 three times daily because any more than that seems to be ineffective.  As for dosing, I always counsel folks to get as close to every 12 hours, or every 8 hours, as you can.  It's the rare ferret that demands his dose at exactly the minute, and if your ferret is that unstable there might be something else going on.

Now keep in mind insulinoma will have ups and downs.  I see critters get into downward spirals routinely and it usually involves not eating frequently enough, rather than a lack of meds.  While you can increase critters pred here, I would also be offering food several times per day to make sure he has something going in regularly and isn't sleeping overlong between meals.

As for the diaxozide, I'm afraid I've never seen it do a thing so I wouldn't consider using it.

I generally don't see problems with ulcers caused by pred, but of course I do give with food or nutrical. Bob Church gave a wonderfully detailed explanation why nutrical is safe to use with ferrets years ago (it's like a complete meal going in, not a candy bar) but of course I didn't save that message!  Anyway, I've used 'cal for years as a booster for critters with insulinoma, just to make sure they have something going in at pred time and then perhaps throughout the day as an extra boost o' food going in.

And no matter what you do, critter will have ups and downs and need your continued management.  But I think here I'd try increased twice daily dose, plus manage his food intake to boost, give him a week to adjust to all that and then see where he is before you go to the three times daily.
